DUE TODAY: Tdap booster documentation for all 7th grade students 2017-2018

What
DUE TODAY: Tdap booster documentation for all 7th grade students 2017-2018
When
8/1/2017

• All students entering into 7th grade are required to show proof that they received a pertussis vaccination (Tdap) to attend school.
• Students who do not meet this requirement will not be permitted to attend school and will be sent home until such proof is received. Under current law, schools do not have the option of offering a grace/extension period.
• Unvaccinated adolescents should get their pertussis vaccination (Tdap) NOW, along with any other vaccinations that they are due to receive, to help them stay healthy and in their classroom.
• Vaccinated students should provide proof that they received a Tdap dose.
• Because Td vaccine does not prevent pertussis, a record that reads “Td” or “Td/Tdap” will not meet the pertussis vaccination school requirement.
• To meet the requirement, students must have received a Tdap shot on or after their 7th birthday. This shot protects them from pertussis, which is commonly called whooping cough, and two other serious diseases, diphtheria and tetanus.
• If students have proof that they already received a Tdap vaccination (on or after their 7th birthday), they do not need another Tdap vaccine to attend school. Please send documentation from the pediatrician or clinic which administered your child’s booster.
• A signed note/letter with name and address on letterhead from your pediatrician, or completed yellow immunization card, are acceptable forms of proof.

►Please bring or mail your student’s proof of TDAP BOOSTER on or before August 1st.
to Mrs. Borjas. Remember, students who do not meet this requirement will not be permitted to attend school and will be sent home until such proof is received.
